DENTAL OFFICE CLOSING This shall constitute notice that the dental practice known as Coast Dental North Druid Hills owned by Coast Dental of Georgia P.C., and located at 2484 Briarcliff Rd NE, Suite 29, Atlanta, GA 30329, is closing effective April 30, 2025. All patient dental records located at Coast Dental North Druid Hills will be transferred to Coast Dental Pleasant Hill located at 1630 Pleasant Hill Rd Suite 200, Duluth, GA 30096. The Coast Dental Pleasant Hill phone number is (877) 880-1212. Patients can request their dental records by sending a written request to the Coast Dental Pleasant Hill address. The patient may be billed for actual costs incurred for copying, mailing, or delivering the records as permitted by law. 4-3, 10-25
