Ft Lauderdale, FL Retina Group of Florida ("RGOF") is notifying individuals who may have been impacted by a data incident. RGOF determined that certain information stored on the RGOF network was accessed by an unauthorized party between November 6, 2024 and November 9, 2024. RGOF is currently conducting a comprehensive review of the information to mail notice to potentially affected individuals once the review is completed. The type of information will likely vary significantly by individual but may include name and one or more of the following: address, telephone number, email address, date of birth, Social Security number, driver's license/state ID number, treatment/diagnosis information, prescription information, provider name, medical record/patient ID number, Medicare/Medicaid ID number, health insurance information, and/or treatment cost.. In an abundance of caution, RGOF is offering individuals access to credit monitoring and identity protection services. For more information or to enroll in these services, individuals should contact the company's assistance line at 833-799-4243, Monday through Friday, from 8:00 AM to 8:00 PM ET, excluding holidays. It is always a good idea to remain vigilant against incidents of identity theft and fraud by reviewing credit reports/account statements and explanation of benefits forms for suspicious activity and to detect errors.
